ADULT ONLINE CONSENT TO PARTICIPATE IN A RESEARCH STUDY
Material Objects and Mass Production Techniques in Santería in Miami
PURPOSE OF THE STUDY
You are being asked to be in a research study. The purpose of this study is to better understand the experiences and opinions of artists, botánica owners, online sellers, practitioners, and community members who produce, sell, or buy material objects. Specifically, this study will focus on the use of mass production techniques and global trade of Santería items in Miami.
NUMBER OF STUDY PARTICIPANTS
If you decide to be in this study, you will be one of twenty people in this research study.
DURATION OF THE STUDY
Participation will require at least one interview, and may consist of up to three follow up interviews. The initial interview will take approximately two hours, and follow up interviews may range from short five-minute chats or additional two-hour interviews. The research process may continue for up to two years, or approximately September of 2019.
PROCEDURES
If you agree to be in the study, we will ask you to do these two things:

We will ask you some questions about your experiences or knowledge about making, selling, using, or delivering Santeria objects.

Our talk will be recorded using this audio recorder [show digital recorder]. This recording will help us remember what we talked about. We need to record the talk so that the talk can be typed up. You cannot be a part of the study if you do not wish for our talk to be recorded.

Alternatively, should in person interviews be unavailable, participants may elect to participate through secured online platforms (such as Skype). These online interviews will follow the same guidelines as in person interviews, and will be recorded using an audio recorder for transcription.

RISKS AND/OR DISCOMFORTS
There are two minimal risks to you participating in this study. One involves maybe talking about past things that might make you feel uncomfortable. I will never ask you to share things that make you feel uncomfortable. At any moment, you may end this interview without telling me why. If any question is too personal, you do not have to answer or you may ask to move on to a different question.
The second small risk to you is loss of confidentiality. We have taken many steps to keep your identity private. If you agree to participate in the interview, we will agree on a fictitious (fake, false or assumed) name. Any document about this interview will only use this fictitious name, which will keep your identity private. Your real name will not be written, stored, or published anywhere.
BENEFITS
You will not be paid or get anything for being in this study, but we hope our work will add to the research on current production of material objects in Miami.
ALTERNATIVES
You do not have to take part in this study.
CONFIDENTIALITY
The records of this study will be kept private and will be protected by law. In any sort of report, we might publish, we will not include any information that will make it possible to identify you.
The audio recording will only have your fictitious name. After the interview, the recording will be securely stored in a password protected file. Later, a member of our team will type up our talk on a computer. All the study files will be stored in a password protected file that only the research team will have access to. However, your records may be reviewed for audit purposes by authorized University or other agents who will be bound by the same rules of confidentiality.
Once we have typed up the audio file, we will destroy the audio recording. However, we will keep the typed papers of our talk in a private secured place for future research.
If you decide to withdraw from the study we will destroy all your recordings and records, using your fake name to identify the materials. You should remember this fictitious name in case you need to contact us in the future, since we will not have anything with your real name.
SUBJECT MATTER EXPERTS & ACADEMIC EXPERTS
Subject matter and academic experts may opt to waive confidentiality and fictitious name option, and have their name and published works acknowledged in any published work that may result from this study.
COMPENSATION & COSTS
You will not get paid to do this and we will not charge you anything either.
RIGHT TO DECLINE OR WITHDRAW
Your participation in this study is voluntary. You are free to participate in the study or withdraw your consent at any time during the study. Your withdrawal or lack of participation will not affect any benefits to which you are otherwise entitled. The investigator reserves the right to remove you without your consent at such time that they feel it is in the best interest.

EL OBJETIVO DEL ESTUDIO
Usted a sido invitado a participar en un studio investigative. El objetivo del miso es comprender mejor las experiencias y opiniones de artistas, propietarios de botánicas, vendedores en línea, practicantes y miembros de la comunidad que producen, venden o compran objetos de Santería. En concreto, este estudio se centrará específicamente en el uso de técnicas de producción en serie y el comercio global de artículos de Santería en Miami.
EL NÚMERO DE PARTICIPANTES DEL ESTUDIO
Si decides serás una de veinte personas en dicho estudio.
EL DURACIÓN DEL ESTUDIO
La participación requerirá al menos una entrevista, y puede consistir en hasta tres entrevistas de seguimiento. La primera entrevista tomará apróximadamente dos horas, y las entrevistas de seguimiento pueden variar desde charlas cortas de cinco minutos o entrevistas adicionales de dos horas. El proceso de investigación puede continuar hasta por dos años, o apróximadamente hasta septiembre del 2019.
LOS PROCEDIMIENTOS
Si acepta participar en el estudio, le pediremos que haga estas dos cosas:

Le haremos algunas preguntas acerca de sus experiencias o conocimientos sobre cómo hacer, vender, usar o entregar objetos Santeria.

Nuestra charla será grabada usando este grabador de audio [mostrar la grabadora digital]. Esta grabación nos ayudará a recordar lo hablado con usted. Necesitamos grabar la charla para que la conversación pueda ser escrita posteriormente. Usted no puede ser parte del estudio si no nos permite que la conversación sea registrada.

Otra alternative seria que si no pudieran hacerse la entrevistas personalmente, los participantes pueden optar por participar a través de plataformas en línea seguras (como Skype). Estas entrevistas en línea seguirán las mismas pautas que las entrevistas personales y se grabarán usando un grabador de audio para la transcripción.
LOS RIESGOS Y / O INCOMODIDADES
Existen dos riesgos mínimos para que usted participe en este estudio. Uno implicaría tal vez hablar de cosas pasadas que podrían hacer que usted se sienta incómodo. Nunca te pediré que comparta cosas que le hacen sentir incómodo. En cualquier momento de nuestros convesación, usted puede terminar esta entrevista sin decir el por qué. Si alguna pregunta es demasiado personal, usted no tiene que contestarlo o usted puede pedir pasar a una pregunta diferente.

El segundo pequeño riesgo para usted sería la pérdida de confidencialidad. Hemos tomado las medidas necesarias para mantener su identidad en privado. Si acepta participar en la entrevista, acordaremos un nombre ficticio (falso). Cualquier documento sobre esta entrevista solo usará este nombre ficticio, el cual mantendrá su identidad en privado. Su nombre real no será escrito, almacenado o publicado en ningún lugar.
LOS BENEFICIOS
No se le pagará ni obtendrá nada por estar en este estudio, pero esperamos que nuestro trabajo se agregue a la investigación sobre la producción actual de objetos relacionados con la Santería en Miami.
LAS ALTERNATIVAS
No tiene la obligación que participar en este estudio.
LA CONFIDENCIALIDAD
Los registros de este estudio se mantendrán en privado y estarán protegidos por ley. En cualquier tipo de informe que pudieramos publicar, no incluiríamos ninguna información que permita que usted sea identificado.

La grabación de audio sólo tendrá su nombre ficticio. Después de la entrevista, la grabación se almacenará de forma segura en un archivo protegido con una contraseña. Más tarde, un miembro de nuestro equipo escribirá nuestra charla en una computadora. Todos los archivos de estudio se almacenarán en un archivo protegido por una contraseña a la cual sólo el equipo de investigación tendrá acceso. Sin embargo, sus registros pueden ser revisados ​​con fines de auditoría por la Universidad autorizada u otros agentes que estarán sujetos a las mismas reglas de confidencialidad.

Una vez que hayamos mecanograficado el archivo de audio, destruiremos la grabación. Sin embargo, guardaremos los papeles mecanografiados de nuestra charla en un lugar seguro para la investigación futura.
Si decide retirarse del estudio, destruiremos todas sus grabaciones y registros, usando su nombre falso para identificar los materiales. Usted debe recordar este nombre ficticio en caso de que necesite ponerse en contacto con nosotros en el futuro, ya que no vamos a tener nada con su nombre real.
EXPERTOS ACADEMICOS Y/O CONOCEDORES DEL ASUNTO
Lod conocdores del asunto y/o los expertos académicos pueden optar por renunciar a la confidencialidad y a la opción de usar un nombre ficticio, para tener su propio nombre y sus publicados ya reconocidos en algua otra obra publicada que pueda resultar de este estudio.
COMPENSACIÓN Y COSTOS
No se le pagará ni cobrará por hacer esto.
DERECHO A NEGARCE O RETIRARSE
Estar en este estudio es voluntario. Usted es libre de participar en él o de retirar su consentimiento en cualquier momento durante el mismo. Retirarse, o no participar, no afectará los beneficios a los que usted tiene derecho. El investigador se reserva el derecho de retirarlo a usted sin su consentimiento en el momento que sienta que es para el mejor interés del estudio.
